Facial sagging: why it happens and how to regain firmness

Dr. Svitlana Lazenko Published on 20 July 2026 6 min read

Facial sagging is usually noticed little by little. It is not a specific wrinkle or a spot, it is a feeling that the face has lost firmness, that the contour is no longer as defined as before. Many people notice it first in photos, or at the end of the day, when the skin looks more “dropped”.

Why the face loses firmness

Skin firmness depends mainly on two proteins: collagen and elastin. They provide support and the ability for skin to bounce back. From a certain age, the body produces less and less collagen, and what exists gradually breaks down.

At the same time, there is a loss of volume in the deeper structures of the face, including fat and bone. It is as if the support beneath the skin were shrinking. As a result, the skin, once stretched neatly over that structure, starts to have excess and to droop.

Factors that speed up this process:

  • Sun exposure over a lifetime
  • Smoking
  • Large weight fluctuations
  • Genetics
  • Ongoing stress and lack of sleep

Where it shows most

Facial sagging tends to appear first in specific areas:

  • The jawline, which loses definition
  • The cheeks, which drop slightly
  • The area around the mouth, with the lines running down from the corners
  • The neck, often forgotten, but one of the first to give it away

How firmness can be regained

The good news is that, in mild to moderate stages, there are several ways to work on sagging without surgery. The approaches fall, simply put, into two ideas that are often combined.

Support. PDO threads act as scaffolding within the skin, helping to reposition tissue that has dropped and to create a firming effect. Beyond the support, they stimulate collagen production in the treated area.

Stimulate. Biostimulators work from within, encouraging the skin itself to produce more collagen over time. The result is not immediate, it is progressive, which tends to give a very natural effect.

In practice, the choice and the combination depend on the degree of sagging, the area and each person’s goal. There is no single recipe: there is an assessment.

When surgery makes more sense

It is important to be honest about the limits. Non-surgical approaches give excellent results in early and intermediate stages, but they do not replace surgery when sagging is pronounced.

A good doctor tells you this clearly. Promising surgical results without surgery helps no one. The role of the assessment is precisely to understand where you are in the process and what is realistic to expect.

What helps prevent it

Beyond treatments, some habits protect skin firmness over time:

  • Daily sun protection, every day of the year
  • Not smoking
  • Keeping a stable weight
  • A skincare routine suited to your age
  • Looking after your sleep and diet

In short

Facial sagging comes from the loss of collagen and support in the structures of the face, and shows mostly in the contour, cheeks and neck. In mild to moderate stages, there are ways to regain firmness naturally, by supporting and stimulating the skin. In more advanced stages, surgery may be the right answer.

If your face feels less firm and you would like to understand what makes sense in your case, book an assessment consultation. We will talk you through the options honestly, without promising what is not realistic.

Everything you need to know

Not always. In mild to moderate stages, there are non-surgical approaches that help improve firmness and stimulate collagen. In more advanced cases, surgery may be the most suitable option. A medical assessment is what guides the decision.

Some approaches give a more immediate lifting effect, others work by stimulating collagen and show results gradually, over weeks. They are often combined, precisely to bring the two together.
